Sequoia’s Adult Rehabilitation Services are designed to support individuals diagnosed with severe and persistent mental health conditions in building the skills necessary for recovery and leading meaningful, independent lives. These services are primarily community-based, allowing us to meet clients where they are and tailor support to their unique needs and goals.

Intensive Case Management Services
Sequoia’s Intensive Case Management Services are designed to support individuals with severe mental illness who require comprehensive, community-based assistance. These services aim to reduce inpatient hospitalizations and interactions with law enforcement, while enhancing each client’s ability to live independently in the community of their choice.
Our approach is tailored to individuals who may not respond well to traditional office-based therapy. By delivering care directly in the community, we are able to meet clients where they are and provide the level of support necessary for long-term stability and recovery. Services are highly coordinated and collaborative, ensuring continuity of care across providers and systems. Services provided include:

Integrated Co-Occurring Services
Sequoia provides Integrated Co-Occurring Disorders (ICOD) services for both adults and adolescents, designed to address the complex and interconnected needs of individuals experiencing both mental health challenges and substance use or behavioral addictions. Our ICOD program offers a holistic, client-centered approach that integrates mental health and addiction treatment into a cohesive care plan, recognizing the importance of treating both conditions simultaneously for more effective outcomes.
Our team is equipped to support individuals struggling with a range of substance use disorders and behavioral addictions, including gambling, nicotine, pornography, and internet use. While Sequoia does not offer specialized treatment for eating disorders, we are able to conduct comprehensive assessments and provide appropriate referrals to trusted external providers when needed. ICOD services include:

BHRN: Low-Barrier Substance Use Treatment
Sequoia offers low-barrier substance use treatment through the Behavioral Health Resource Network (BHRN), providing essential support at no cost to individuals who are uninsured or underinsured. These services are available to anyone seeking help with substance use, regardless of insurance status, housing situation, or immigration status. In addition to core treatment services, eligible BHRN clients may access supported employment assistance to help stabilize and rebuild their lives.
To further reduce the risks associated with substance use, Sequoia distributes free Narcan and conducts street outreach to engage individuals who may be actively using substances, offering support and overdose prevention resources in real time. BHRN services include:

Child & Family Services
Sequoia’s Youth and Family Services are designed to support children, adolescents, young adults (ages 6 and up), and their families in navigating mental health and addiction challenges. We offer a wide range of evidence-based treatments aimed at building resilience, strengthening family relationships, and promoting emotional well-being.
Our approach incorporates proven, evidence-based practices, including Cognitive and Dialectical Behavioral Therapy (CBT/DBT), and Motivational Interviewing, to provide personalized, effective care tailored to the unique needs of each youth and family. In collaboration with local high schools, we deliver school-based interventions to make services more accessible and integrated into students’ daily environments. Services provided include:

Sequoia operates five licensed residential treatment homes in Washington County, providing structured, 24-hour care for individuals transitioning from more secure settings. These homes offer a safe and supportive environment where residents can develop daily living skills and connect with resources to support successful community integration.
Licensed by the Oregon Health Authority and governed by Oregon Administrative Rules, each home is staffed with trained professionals who provide continuous support and treatment. Access to these homes is coordinated through a state-wide referral and screening process, typically involving an Enhanced Needs Care Coordinator, who assists with the referral and screening process. Residential Treatment services include:
